Medical Records
Mount Sinai Medical Center protects your healthcare information in compliance with federal laws, State requirements and HIPAA regulations. Medical records can only be released with proper authorization from the patient or the patient’s legally authorized representative.
Your Medical Records Online Through MyChart
Your medical record for both physician practices and hospital records can be released and downloaded from your secure MyChart patient portal. Click HERE to login to your MyChart account.
- Use the Visit Records section to download what is already available to you for physician office visits, ED, and Hospital Admissions.
- Use the Request Medical Records to request specific legal hospital records that are not available to you in MyChart, to be released to your MyChart account in the form of a PDF, for you to download and review.

ONLINE Hospital Medical Record Request
Requesting your medical records online is an easy, fast, and secure way to receive your information. We recommend online medical requests whenever possible. Just use the button below and follow the prompts.
IN PERSON/VIA MAIL or EMAIL Medical Record Request
If you are unable to complete your records request online, records may also be requested by visiting the Health Information Department to fill out an authorization form, mailing/faxing an authorization form, or emailing an authorization form to the HIM Office.
All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient’s legally authorized representative.

Picking up Your Hospital Medical Records
Medical records are to be picked up in person at the Health Information Management Department. Please find the address above.

Monday – Friday, excluding holidays
8:30a.m. – 4:00p.m.
Mount Sinai Employed Physician Practice Medical Records
All requests for physician practice records must be submitted in writing and must be dated and signed by the patient or the patient’s legally authorized representative.
The form must be submitted to the physician practice where your records are kept.
The practice will contact you once records are available.
